Please call your insurance and ask for your mental health benefits for outpatient, office setting. Ask if Transformation Behavioral Health in Woodstock Illinois is in-network with your plan. If your insurance company needs more information, please contact us.
You have an option of paying copay, co-insurance and deductible amounts by cash, check, health savings account, or credit card. With a credit card, patient cost-sharing amounts can also be paid through your patient portal. These amounts are payable at the time of service.
If we are out-of-network with your health insurance, you will have to pay for services yourself. You can contact your insurance and ask if your out-of-network benefits will apply. Those benefits often have very high deductible and co-insurance amounts. You might also choose to ask your insurance if there is an option for a single-case agreement.
We are NOT in-network with Blue Cross Community Care, Meridian, and other managed Medicaid policies.
We are NOT in-network with Beacon (Value Options), Magellan, Medicaid, UHC Medicare Advantage or Humana Medicare Advantage.

*** A note about HMO and EPO plans. Please remember that when you choose an HMO or EPO plan, you are agreeing to see only the narrow range of providers who are in that specific HMO or EPO. Those providers who are PPO providers are excluded from HMOs. If you have chosen an HMO or EPO plan and wish to see a PPO provider, you will pay for services yourself. HMO and EPO plans will not even accept claims from PPO providers. We are not able to submit claims for you if you have an HMO or EPO plan.

*** A note about "Medicare Advantage" plans. These plans are NOT Medicare. They are commercial insurance companies that have purchased the ability to provide health insurance to Medicare-eligible persons. They often have high patient copay amounts and exclude some services. Starting Jan 2025, many Medicare Advantage plans are excluding more services that were previously covered. A Medicare provider may or may not be in-network with one of these specific commercial insurance plans. If you have a Medicare Advantage plan, you are responsible for contacting your insurance and finding out if and how services with Transformation Behavioral Health will be covered.
